Meeting Minutes

November 22, 2005

The October minutes were approved.

Accident and injury reports were reviewed and discussed.

Hall asked a question about who is responsible for filling out the Supervisor’s Accident and Injury Report (SAIR).  Is it the employee or the supervisor?  He was concerned that at times a supervisor disagrees and feels that an accident could not have occurred the way it was reported, and this is reflected on the SAIR.

Kay said that the best option would be for the employee and supervisor to complete the SAIR together. She pointed out that the supervisor and the employee has the opportunity to provide additional information related to the incident on the lower part of the form.  Michelle Gillette recommended that employee and/or supervisor contact Environmental Health and Safety for assistance.  Gary also pointed out that a safety hazard may also be reported to a department’s safety committee.

New Business

Chairperson Election

Mark Maguire nominated Michelle Wygle.  The nomination was seconded, and the vote was unanimous in electing Michelle as the 2005-06 chairperson.

New Member Orientation

Kay will hold a training session for new committee members prior to the next meeting.

Inspection Subcommittee Report

During the last inspection four safety hazards were found in Bean and fifteen in Carson Hall. 

Ernie asked whether confirmation is received after the building management has taken corrective action.  The Inspection Subcommittee decided to add a requirement to future memos that list the safety hazards.  These memos will request a response within approximately thirty days indicating what their plan is to correct the safety hazards.

The next Safety Advisory Committee meeting will be held on December 15, 9:00 – 10:00 a.m. in the EMU Rogue Room.
